Have you been diagnosed with a chronic disease such as diabetes? Are you considered “high risk” because of factors like heart disease, obesity or high blood pressure? If so, you’re probably worried or even scared to start an exercise program. Sound like you? Well, good news. Hancock Wellness Centers have just the thing to help you get on track to a healthier life: the Physician Referral Program.
Chronic diseases impact a whopping 133 million Americans, or 40% of the country, with that number growing every year. That’s some serious stuff. Some diseases, such as obesity, diabetes and cardiovascular disease, keep people from performing daily tasks and making healthy choices. As a branch of Hancock Regional Hospital, Hancock Wellness Centers aim to provide a resource for those within our community who want to make better choices. That’s why we offer the Physician Referral Program.
What can we do for you for $60?
This program, considered a cornerstone of our wellness centers, provides access to supervised fitness for all demographics at an affordable rate of $60 for 60 days. For the duration of the two-month period, clients can participate in supervised exercise up to three days a week, with each session lasting about 30-60 minutes.
Patients may be referred by a physician, physical therapist, dietician, diabetes educator or even use self-referral. Once referred, patients will meet our team for a one-on-one initial assessment. This is when you discuss wellness goals as well as any health challenges or concerns. With a personalized wellness plan in place, patients are then able to go at their own pace to reach their goals, whether that’s losing body fat, increasing cardiovascular strength, improving balance or regaining their ability to participate in activities they love.
Our medically certified exercise specialists are there to guide patients throughout the entire journey while under the watchful, caring eyes of Hancock Health physicians, who are dedicated to helping patients improve their quality of life.
Client integration with the Physician Referral Program
“We understand that everyone’s health and wellness journey is different. We take the guesswork out of fitness and nutrition and prescribe programs that lead to healthier and happier clients,” says exercise specialist Abby Glazier. “Our integration with the hospital’s electronic medical records system also keeps the doctor up to date with the client’s progress from start to finish. This positions us to be your one-stop-shop for all things health and wellness.”
Is it time to take back your life? Exercise will positively impact a host of chronic diseases as well as mental health challenges. If you’re ready to join us, talk to your physician about getting a referral for our safe and innovative program. We hope to get you moving and enjoying life again in no time!
